Patient FAQ’s
What do I need to do if I’m a new patient?
On the menu, if you click on the Forms tab, you can complete the set of new patient forms before arriving to your appointment. Make sure you bring medication list and any other information the provider needs to know.
What insurance do you accept?
Please be advised that our practice no longer participates with insurance plans. If you have out of network benefits we will be happy to give you a claim for you to submit.
Are cosmetic procedures covered by my insurance?
Unfortunately, cosmetics are self-pay and are not covered by insurance.
What are your office hours?
Monday 9-7 (alternate)
Tuesday 9-2
Wednesday 9-5
Thursday 9-5
Friday 9-4 (alternate)
What should I do to cancel an appointment?
Call within 24 hours prior to your appointment to cancel or else you will be subject to a $50 no show fee.
Do you provide phone consulations?
Unfortunately, we do not provide phone consultations. We recommend that you schedule an appointment with a provider and they will be able to assist you.
How do I find out my lab results?
Typically our staff with contact you if there are any lab abnormalities. However, please call our office on status of lab results.
How do I get a refill?
If you have not been seen in the past 3 months you must schedule an appointment before you can get your refill. Otherwise, please call your pharmacy and let them know which prescription needs to be filled. We will not refill prescriptions over the weekend.
How can I transfer my records to another practice?
On the menu, if you click on the Forms tab there is a form called Authorization to Release Records from another doctor’s office. Please complete that form and we will contact that office for futher information.
What is your refund policy?
Reflect Medical & Cosmetic Center does not allow refunds on any gift card certificates or vouchers. It is the patient’s responsibility to pay for services and product. The patient may not dispute services or products already provided and/or purchased. All sales are final.
Reflect Medical can provide patients with office credit for any products* that have not been opened or used that the patient would like exchanged or returned to the office.
*Upneeq and Latisse are excluded. These two prescription products are not allowed for any type of return/exchange in our office.
